The Hudson Valley Care Coalition is leading the region's new Social Care Network (SCN).

The SCN was started by the Department of Health. It helps Medicaid families with getting food, housing, and transportation.

To apply, families can either visit the coalition's website or their local health care provider. They can take a confidential screening. "If the need is identified, they will be assigned a person to help them navigate the system," says Amie Parikh, CEO of the Hudson Valley Care Coalition.

The organization says there are many services available for families in need. These services include access to healthy meals, eviction prevention, safe housing repairs, transportation to social services, and nutrition counseling. "There are a lot of services that people can qualify for," says Parikh.
