Two Ulster County men were indicted in separate cases, both accused of killing their fathers.
Brian Roberts, a 26-year-old from Lake Katrine, is accused of killing his father, 57-year-old Anthony, earlier this month.
The DA's office said Roberts has struggled with mental health his whole life and his family tried to get help many times but were often left to navigate services in the community alone.
The DA's office said 29-year-old John Arceri, of Kingston, strangled his father, Ram, in February in the Town of Rosendale.
Arceri's family said he has suffered mental health issues much of his adult life. They also said the county's mobile mental health unit has been at the home several times, including 15 minutes before the homicide.
According to the DA's office, Arceri was taken to the hospital the night before after a disturbance call at a Kingston business.
He was released the same night.
The DA's office said both cases are examples of a broken mental health system that offers help too late.
A deputy chief assistant DA said it will take a multi-agency effort to implement changes needed to prevent these crimes.
